B. Outside City Limits Fuel Surcharge: Additionally, a fuel surcharge is imposed on container accounts that are located outside the city limits. The surcharges are:

(1) Two (2) to five (5) miles outside city limits, add $12.26 per pull in addition to base container rate;

(2) Over five (5) to ten (10) miles outside city limits, add $32.72 per pull in addition to base container rate;

(3) Over ten (10) to fifteen (15) miles outside city limits, add $53.16 per pull in addition to base container rate;

(4) Over fifteen (15) to twenty (20) miles outside city limits, add $73.61 per pull in addition to base container rate.
